From this page you can download various forms to manage your account with Bridgend Lifesavers Credit Union.

How to Use these Forms

The following forms are available for download. You will need Adobe Acrobat Reader to view and print them. Acrobat Reader is available, free, from the Adobe web site.

Once you have installed the software, you can download and print any of the forms below. Then simply fill in the required details and send to our Offices in Sarn. Alternatively, please bring the completed forms to the Credit Union Office or to any of our convenient collection points – a list of our Collection points can be found in our Contact Us section.

Application Form

Complete this form to set up a new account (Regular Savings or Junior Savings Account). Please note that you will be required to provide proof of identity when setting up a new account. Please click on Join Us for more details of how to join your local Credit Union.

Use this form to notify us of a change of address, name or nominated person.

Child Trust Fund Membership Form

To set up a Child Trust Fund with Bridgend Lifesavers Credit Union, download the Child Trust Fund Membership Form, together with a Membership Application form and return to us with your CTF voucher and a copy of your child’s birth certificate.
